Here’s a sample of Section E of the 4473 with detailed instructions below:

32. For Use by Licensee. You may put whatever you wish here – this is a good spot for another employee to put their initials after they have double-checked the form for accuracy prior to firearm transfer. This is the ONLY spot on this form that you may put information or marks, which aren’t direct responses to a particular section. I’ve seen many FFLs put marks in the margins or the top of the form as their own internal codes – do NOT do this.
33.Trade/Corporate name, address, and FFL number of transferor/seller. This is an easily forgotten section. We recommend getting a stamp with this information on it to easily stamp each form 4473.
You may stamp these in advance, however, be careful doing this because it is easy to forget about this section, and you may not notice when you’ve run out of stamped copies and are accidentally leaving this section blank. DO NOT USE YOUR FULL FFL NUMBER HERE. Instead, use the abbreviated FFL number as allowed by ATF: your first three digits and the last five: 1-22-55555. The reason for this is the omitted digits are your license type and expiration date (which changes upon FFL renewal). If you get a stamp with the full number, then you’ll need a new stamp once your FFL renews, and you run the risk of accidentally stamping the old number.
34. Transferor’s/Seller’s Name. WAIT TO FILL THIS OUT UNTIL THE FIREARM IS ACTUALLY BEING TRANSFERRED. Whoever is completing the 4473 on behalf of the FFL will print their name here.
35. Transferor’s/Seller’s Signature. WAIT TO FILL THIS OUT UNTIL THE FIREARM IS ACTUALLY BEING TRANSFERRED. Whoever is completing the 4473 on behalf of the FFL will sign the form here.
36. Date Transferred. WAIT TO FILL THIS OUT UNTIL THE FIREARM IS ACTUALLY BEING TRANSFERRED. Enter the date that the firearm was transferred.
