Yes.  The 4473 is a federal form required by federal law.  So no matter what state documentation may be required, the 4473 is always required.

A 4473 is always required if you sell or dispose of, temporarily or permanently, any firearm to any person, other than another FFL.  The only exception to this is the return of a repaired firearm to the person from whom it was originally received.
