If you’ve searched everywhere possible and you are absolutely certain the form is lost:

Double check with the customer to make sure they didn’t take it by mistake.
If still missing, immediately contact the ATF field office for your business activity location and let them know.
Complete a new 4473 form for the customer but attach a statement to it explaining that it’s a replacement because the original was lost.
File the replacement 4473 as you normally would with your records.
